摘要
目的 探讨如何解决常规钡灌肠诊断低位先天性巨结肠(Hirschsprung's disease,HD)的不足,提高影像学存此疾病的诊断价值。 方法 利用低位HD大肠积气,灌入气钡造影浓度的钡液形成气钡影像,并充分利用气体和沿途的流体动力学特征,设计倒立位方法,使直肠、乙状结肠充分拉直展示。 结果 242例低位HD一次性造影成功率达95%以上,狭窄段显示209例,移行段显示202例,均显示有代偿扩张段,代偿扩张段与移行段或狭窄段比值≥1.5:1。 结论 气钡造影倒立位法渗断低位HD具有方法简便,一次成功率高,病变显示好,测量数据较准确等优点。
objective In order to improve the value of medical imaging in diagnosing the low position Hirschsprung's disease(HD), we created a new method which overcome the shortcomings of traditional diagnostic methods such as barium enema(BE). Methods Barium of proper concentration was perfused into colon. A double contrast barium image was got by means of large quantity of gas in the colon of low- position HD patients. By using the principle of gas and liquid dynamics, we designed the Up- Side-Down method to make rectum and sigmoid colon fully straightened. Results This method was successful in over 95% of 242 low-position HD patients, getting image in once. Narrow segment was displayed in 209 cases and transition segment in 202 cases. Expanded compensation was displayed in both above. The ratio of the compensation expanded segment and the former two was 1.5:1 or over 1.5:1. Conclusion Low-Position HD can be diagnosed by Up-Side-Down method of double contrast barium-enema examination conveniently. It could clearly and accuraly reflect the pathologic changes.
