抓髌器外固定治疗髌骨骨折的生物力学机制初探

摘要 目的 从髌骨及其骨折的生物力学特征的角度探讨抓髌器治疗髌骨骨折的治疗机制。方法 抓髌器治疗 2 4例髌骨骨折。左侧 15例 ,右侧 9例。横断骨折 18例 ,上极骨折 1例 ,下极骨折 3例 ,粉碎性骨折 2例。均在伤后 4 8h内行四爪抓髌器治疗。术后 3d开始功能锻炼。结果 术后 14d屈膝达 90°以上者 2 3例 ,占 95 .8%。随访时间最长 2年半 ,最短 5个月 ,平均14个月。根据陆裕朴评定标准 ,优 2 0例 ,良 3例 ,可 1例 ,优良率 95 .8%。结论 抓髌器治疗髌骨骨折 ,符合生物力学规律 ,其固定可靠 ,操作简单 ,效果满意。 Objective To investigate the treatment principle of patellar fracture of patellar talon in characteristic biomechanics of patellar and its fracture.Methods 24 cases patellar fracture, among them,15 left and 9 right, 18 traverse fracture, 1 upper pole fracture and 3 lower pole fracture, were had treated with patellar talon in 48 hours, dirigation after three days. Results The follow-up period were 5 months to 30 months with average of 14 months. The knees angle of flexing had reached more than 90° for 23 cases after 14 days. All patients were evaluated with the criteria of LU Yupu's standard. The recovery of knee joint function was excellent in 20 cases, good in 3, fair in 1. Excellent and good results were reached in 95.8% of the patients.Conclusion The treatment of patellar fracture with patellar talon conforms to biomechanics regulation. It has the advantage of simple manipulation, rigid fixation, and effective satisfaction.
