摘要
目的 探讨高龄食管癌患者的外科治疗方法。方法 回顾性分析我院自 1986～ 2 0 0 3年间 6 4例70岁以上食管癌患者的外科临床资料。结果 高龄食管癌患者院前误诊率高 (5 6 % )合并症多 (5 9% ) ,肿瘤切除率 93% ,术后并发症发生率 2 6 % ,围手术期病死率 6 %。结论 手术切除是高龄食管癌患者的最佳方法 ,但早期诊断 ,早期治疗 ,合理处理并存病 ,加强围手术期营养支持 ,适当的麻醉及手术方式 ,有效的预防和处理并发症是提高疗效的关键。
Objective To evaluate the surgical treatment of elderly patients for esophageal carcinoma. Methods Retrospective analysis on the experience of surgical treatment for esophageal carcinoma in 64 patients over 70 years of age from 1986～2003.Results Misdiagnosis rate was high(56%), Most of the patients had other diseases (59%) tumor resection rate 93%,post operation complication rate was 26%, operation mortality 6%.Conclusion The operation is an ideal way to treat the elderly patients with esophageal carcinoma. Early diagnosis and treatment、 correct management preexisting diseases,the nutrition support in perioperative、 aprropriate choice of anesthesia and operation procedure,effective method for the prevention and treatment of complication,are keys to improve effect on the elderly patients with esophageal carcinoma.
