神经性厌食症40例临床分析

A Clinical Analysis on 40 Cases of Anorexia Nervosa

目的 探讨神经性厌食症 (anorexianervosa ,AN)的临床特点 ,提高对该病的诊治水平。方法 对住院治疗的 40例AN患者进行回顾性分析。结果 本病青年女性多见 (95 % ) ,平均年龄 2 0 8± 8 4岁。常见诱因为学习或工作压力大(5 5 % ) ,临床上以厌食 (10 0 % )、消瘦 (10 0 % )、闭经或月经稀发 (70 % )为主要表现 ,伴有多种内分泌功能紊乱 ;5 7 5 %的患者被误诊为甲状腺功能减退症或非溃疡性消化不良等疾病。所有患者入院后均给予心理治疗、促胃肠动力药及营养支持治疗。平均住院 2 1± 13d ,除 1例患者入院 3d后自动出院外 ,其余患者食欲基本恢复正常 ,体重增加 (1～ 5kg) ,全身情况改善。结论 AN仍是一个被人们认识不足的疾病 。

Objective To analyse the clinical characteristics of anorexia nervosa (AN) and to improve the diagnosis and treatment of AN Methods Forty medical records of in-patients with AN from December 1989 to July 2003 were carefully and retrospectively studied Results ① AN often occurred in younger female patients(95%), average age was 20 8±8 4 years; ② Most predisposing factors of AN were serous stress on study or work (55%); ③ The major clinical manifestations of these patients were anorexia (100%), pathologic leanness (100%), amenorrhea-oligomenorrhea (70%) and low metabolic state, etc ; ④ The misdiagnosis rate was 57 5% before admission; ⑤ The symptoms and signs gradually improved in 39 cases with dietary and mental health treatment after 3 weeks (21±13 days) Conclusion AN at present remains to be an underdiagnosed and undertreated disorder and more attention should be paid to it for improving the accuracy of diagnosis and adequacy of treatment
