1979年以来,对118例病人,其中颈内动脉一海绵窦瘘88例、动静脉瘘8例、假性动脉瘤22例采用了血管内治疗:①可脱性球囊栓塞64例:②弹簧圈栓塞19例;③钨丝螺旋圈栓塞15冽;④肌瓣“放风筝”法治疗6例:⑤眼上静脉穿刺注入栓塞材料2例;⑥瘤腔直接穿刺注入栓塞材料2例;①气囊导管暂时阻断动脉腔内血流与血管重建相结合治疗10例.2例颈内动脉-海绵窦瘘病人术后出现偏瘫,在半年内恢复,余皆治愈.
The treatment of false aneurysms andarteriovenous fistulas which located near thetrunk is usually difficult. From 1979 to 1995,118cases had been treated by endovascular emboliza-tion. Among them, 88 cases were carotid-fistu-las. 8 cases of arteriovenous fistulas,and 22 casesof false aneurysms. The regimen of treatment in- eluding: ① 64 cases were embolized by the de-tachable balloon; ② 19 cases were embolized bythe spring-coils; ③ 15 cases were embolized bythe tungsten microcoils; ④ 6 cases were treatedby muscle tissue 'fly a kite'; ⑤ Embolic materi-als were injected via superior ophthalmic vein in2 cases; ⑥Embolic materials were directly inject-ed through aneurysmal wall in 2 cases; ⑦Aftertemporary occlusion of blood circulation by bal-loon catheter, repair of the blood vessels hadbeen performed in 10 cases. All of the patientswere cured except 2 cases who had hemiparaly-ses after operation, but recovered 6 months lat-er. Arteriovenous fistula and the cavity of falseaneurysm were embolized by detachable balloon,the lesions were closed up and patency of sup-plied arteries were preserved. The spring-coilsand tungsten micro-coils in the blood may inducethe thrombogenesis and proliferation of fibersand finally cure the lesion. In muscle mass 'fly akite', muscle flap may block up the arteriove-nous fistula and the cavity of false aneurysm.This method is usually simple and safe. It is par-ticularly suitable for the portions of incapablityof direct operation. Embolism with local punc-ture is simple,safe and reliable. After temporaryocclusion of blood circulation by ballooncatheter, repair of the blood vessel can be per-formed. It may control and decrease bleedingduring operation. It is a safe and reliable for aux-iliary method in direct operation. The successfulrepair rate for larger blood vessel injured close totrunk could be increased.
