高血压病左室肥厚对心室收缩期储备功能的影响

Effect of left ventricular hypertrophy (LVH) on cardiac systolic functional reserve in essential hypertension

应用二维超声心动图Simpson法,在尚无临床心衰征象、静息左室射血分数正常的49例高血压病患者中,检测出伴有左室肥厚的28例病人在踏车极量运动时左心收缩功能的反应低于无左室肥厚组(21例)及年龄配对的血压正常对照组(23例)。提示伴有左室肥厚的高血压病患者收缩期储备功能已受损。

The response of left ventricular (LV) ejection fraction (EF) during maximal upright bicycle exercise was measured by 2-dimensional echocardiography in 49 male uncomplicated es- sential hypertensive patients with or without LVH and 23 male age-matched normotensive sub- jects (NT). The EF at rest was similar among the 3 groups. The group without LVH (n=21) had a similar EF exercise response (EF increased from 68±7% to 74±5%) as compared with that of NT group(68±5% to 76±5%). The LVH group (n=28) showed a lesser increase in EF with exer- cise (69±6% to 70±5%) and their exercise EF were lower than those of NT (P<0.01). In hypertensive patients, the increase of EF during exercise showed an inverse correlation with LV end-diastolic wall thickness (r=-0.35). The results suggested that the LV systolic functional re- serve was impaired in hypertensive patients with LVH.
