颞浅血管蒂额部岛状皮瓣修复鼻部分缺损

Forehead island flap transfer with superficial temporal vessels and fascia pedicle for the treatment of nasal partial defects

目的 研究颞浅血管蒂额部岛状皮瓣修复鼻部分缺损的方法。方法 自 1992年以来采用颞浅血管蒂额部岛状皮瓣修复鼻部分缺损 17例 ,全层缺损以皮瓣远端折叠法或局部翻转皮瓣法制作衬里。皮瓣最小面积 1.5cm× 3cm ,最大面积 8cm× 10cm。结果 17例中 ,移植皮瓣完全成活 12例 ,经随访 1～ 11年 ,鼻缺损修复后轮廓形态逼真 ,移植皮瓣颜色和周围肤色差别小 ,外形和功能良好。皮瓣供区痕迹不明显。移植皮瓣回流障碍 3例 ,术后局部遗留色素改变。移植皮瓣部分坏死 2例 ,皮瓣部分坏死后 3周 ,用对侧鼻唇沟带蒂皮瓣移植修复。结论 颞浅血管蒂额部岛状皮瓣修复鼻部分缺损 ,是一种良好的选择。

Objective To study the method of repairing the nasal part ial defect using a forehead island flap transplantation with a superficial tempo ral vessels and fascia pedicle. Methods The retrospective inves tigation was performed in 17 cases of the nasal partial defect repaired with the forehead island flap with a pedicle of superficial temporal vessels and fascia since 1992 to 2002, including eleven males and six females with the age of 15 to 69 years. The etiology was burn (9 cases), injuries (6 cases) and nasal tumor (2 cases); The defects located in nasal alar (ten cases) and nasal scar (7 cases ). The size of transplantated skin flap ranged from 1.5 cm×3cm to 8 cm×10cm. Results Seventeen cases of forehead island flap transplantation with a superficial temporal vessels and fascia pedicle were completely survival and achieved aesthetic contouring results in 12 cases after following-up for o ne to eleven years. Meanwhile, three transfer flaps involved postoperative venou s congestion and resulted in changes of the skin color; Two transplantation flap s were partial necrosis. Conclusion The method of forehead isl and flap transplantation with a superficial temporal vessels and fascia pedicle is a safe and potent choice for the reconstruction of partial nasal defect.
