摘要
目的探讨急性哮喘患者血浆白细胞介素18(IL18)、IL16、IL4及γ干扰素(IFNγ)水平的相关性及临床意义。方法急性发作期哮喘患者共60例(重度30例,中度30例),正常对照30例。采用双抗体夹心酶联免疫吸附实验(SELISA),检测血浆IL18、IL16、IL4及IFNγ水平。结果哮喘患者血浆IL16水平显著降低,正常对照组血浆IL16较中、重度哮喘组高3倍以上(P<005);IL18水平降低,但仅中度哮喘组与正常对照组相比有显著性差异(P=0015);IL4水平降低(P=0006),中、重度哮喘组与正常对照组相比,P值分别为0045和0043;IFNγ水平与正常对照组比较无显著性差异(P>005)。相关性分析表明IL18与IL16之间存在显著相关性(r=065,P<005),IFNγ与IL16之间也存在显著相关性(r=066,P<005),IFNγ与单核细胞计数存在相关性(r=039,P<005)。结论IL18、IL16、IL4和IFNγ均可能参与了哮喘病理生理过程,多年反复发作的中、重度发作期哮喘患者可无明显的Th1与Th2细胞因子失衡现象。
Objective To investigate the plasma levels of interleukin 18 (IL 18),IL 16,IL 4 and interferon γ (IFN γ) in acute asthmatics and their clinical implications.Methods A total of 60 acute asthmatics (30 severe and 30 moderate) and 30 healthy subjects were recruited into the study.IL 18,IL 16,IL 4 and IFN γ levels were detected with Sandwich SELISA procedures.Results At acute attack stage of patients with asthma,the plasma levels of IL 16 were at least 3 fold lower than that of healthy subjects ( P <0 05).The plasma levels of IL 18 and IL 4 were decreased in the patients with asthma( P =0 011 and P =0 006).The plasma levels of IFN γ did not change significantly ( P >0 05).There were correlations between the levels of IL 18 and IL 16 ( r =0 65 , P <0 05),IFN γ and IL 16 ( r =0 66, P <0 05),IFN γ and monocyte counts ( r =0 39, P <0 05). Conclution IL 18,IL 16,IL 4,and IFN γ may play rolesin the pathogenesis of asthma and the imbalance of Th1/Th2 cytokines may become obscure in moderate and severe acute asthma with a relatively long history of illness.
