晚期胃癌的治疗现状

胃癌的化学治疗

迄今晚期胃癌化疗作用仍属姑息性 ,传统的化疗方案治疗比最好的支持治疗的中位生存期明显提高 ,随着各种新的抗癌药物的出现 ,晚期胃癌治疗的有效率和生存期有望进一步提高 ,但目前尚无标准的联合治疗方案。

The treatment indication,objective,regimens and future direction of gastric cancer were described in this article.Only 50% of all patients with gastric cancer presented with a respectable disease at diagnosis,and less than 50% of operated patients underwent radical surgery.Furthermore,even after a 'curative' gastrectomy,disease might reoccur in both regional and distant sites in more than 50% of patients.Therefore,adjuvant chemotherapy after complete surgical resection was necessary.Up to the present,patients might have minimal benefit from adjuvant chemotherapy.Patients with advanced gastric cancer would benefit from chemotherapy compared with best supportive care.Downstaging the primary tumor by neoadjuvant chemotherapy has been made to increase the possibility of a successful complete resection.A large United States Intergroup study (INT-0116) showed that chemoradiation combined with gastric resection improved median time to relapse and overall survival.
