摘要
目的探讨脑干肿瘤的外科治疗方法及其注意事项。方法48例脑干肿瘤依据MRI选择不同入路行显微手术治疗,术后给予呼吸、循环功能的对症处理。结果肿瘤全切除25例,次全切除10例,大部切除8例,部分切除5例;术后并发呼吸功能障碍10例,循环功能障碍4例,咳嗽反射减弱和上消化道出血各5例,肌力减弱9例;术后随访35例,神经系统功能明显好转29例。结论术前MRI检查和一定的手术技巧是脑干肿瘤手术治疗取得良好效果的关键,术后监护和并发症的处理可增加手术安全性。
Objective To investigate the surgical treatment of brainstem tumors.Methods In the light of MRI,different approaches of microsurgical removal were selected in 48 brainstem tumors patients.Results Tumors were totally resected in 25 cases,subtotally resected in 10 cases,mostly partially removed in 8 cases and partially removed in 5 cases.Postoperative complications were respiration dysfunction in 10 cases,circulation dysfunction in 4 cases,attenuation of cough reflect in 5 cases,hemorrhage of upper digestive tract in 5 cases and attenuation of myodynamia in 9 cases.The nervous system function recovered obviously in 29 cases among 35 followed up cases.Conclusion Preoperative MRI and surgical technique are the key guarantee of good effects in management of brainstem tumors and postoperative monitoing and management of complications can increase the safety of microsurgery.
