重型脑室出血急性脑积水治疗的研究 被引量:6

Treatment of severe intraventricular hemorrhage with acute hydrocephalus.

摘要 目的探讨重型脑室出血急性脑积水的治疗,以降低延期脑积水的发病率。方法在脑室持续引流及尿激酶灌注的基础上,向腰蛛网膜下腔注入尿激酶。结果经尿激酶治疗后,4例未发生延期脑积水,占80%;1例为高压型脑积水,行“脑室腹腔分流术”,占20%。结论在脑室持续引流及尿激酶灌注的基础上,向腰蛛网膜下腔注入尿激酶,能降低延期脑积水的发生率。 Objective To explore the treatment of severe intraventricular hemorrhage(SIVH) with acute hydrocephalus,in order to decrease the morbidity of delayed hydrocephalus.Methods Infusion of urokinase into the subarachnoid cavity by spinal puncture is based on continuous ventricular drainage and intraventricular infusion of urokinase.Results After urokinase therapy,there were 4 cases without delayed hydrocephalus(80%).There was 1 case with delayed high pressure hydrocephalus and treated by a VP shunt operation(20%).Conclusion Based on continuous ventricular drainage and intraventricular infusion of urokinase,and then urokinase is infused into the subarachnoid cavity by spinal puncture,which can decrease the morbidity of delayed hydrocephalus.
