应用足延长器治疗足缺损9例观察报告

Clinical study on reconstruction of front and middle part of foot defect with foot lengthening apparatus (nine cases observation)

目的探讨前中足缺损的病例,应用自行设计的足延长装置在施加牵伸应力的条件下,使缺损的前中足复合组织再生的可行性。方法对9例(10足)患者采取在残留后足的跟骨、距骨截骨,以15或20mm的克氏针分别多平面穿过截骨后的跟、距骨两侧骨块,并与延长装置连接固定。术后调节装置每天以075～10mm的速率延长撑开器,直至达到预定延长长度。观察对比残足延长前后外形及功能的变化。结果9例病人残足平均延长825cm。再生新足皮肤(包括足底)的感觉、厚韧度及弹性正常;外形接近健侧足长度及宽度;马蹄内翻足畸形及跛行步态矫正满意。结论应用足延长器行前中足缺损延长再造新足,是解决前中足缺损所致足部继发畸形、发育障碍及功能缺陷等问题的有效途径。
