摘要 目的 了解医院高产AmpC酶和超广谱 β 内酰胺酶 (ESBLs)阴沟肠杆菌的流行情况。方法 用改良三维实验检测 86株阴沟肠杆菌中表型可疑产酶的菌株。结果 检出产酶菌 2 6株 ,其中高产AmpC酶 17株 ,ESBLs 4株 ,同时高产AmpC酶和ESBLs 5株 ;产酶菌对多种抗生素的耐药率明显高于不产酶菌。结论 及时准确检测阴沟肠杆菌的产酶株 ,对指导临床合理使用抗生素、减缓对细菌耐药的选择性压力、避免医院感染的发生和流行有重要意义。 OBJECTIVE To study the epidemic of Enterobacter cloacae producing AmpC enzymes and extended-spectrun β-lactamases (ESBLs). METHODS Three-dimensional test was performed to detect the phenotype of 86 screening strains from E. cloacae. RESULTS From 26 strains detected to produce enzymes,17 strains highly produced AmpC enzymes, 4 strains produced ESBLs, and 5 strains produced AmpC enzymes and ESBLs. The drug-resistant rate of antibiotics to AmpC or /and ESBLs positive strains have significantly higher than negative ones. CONCLUSIONS It is important to detect E. cloacae producing β-lactamases properly and rapidly for directing the clinical doctors to use antibiotics reasonably, relieving the selective pressure of bacteria resistance, avoiding the nosocomial infection.
