良性阵发性位置性眩晕的诊断和治疗

Diagnosis and therapy of benign paroxysmal positional vertigo

目的 :探讨良性阵发性位置性眩晕 (BPPV)的诊治方法。方法 :回顾性分析 8例BPPV的临床资料。对所有患者全面采集病史及耳神经学检查 ,并施行Dixt-Hallpike试验及仰卧侧头位试验 ,按BPPV的不同类型分别采用Epley耳石复位法和Lempert翻滚法进行治疗。结果 :8例BPPV中有 5例属于后半规管性BPPV(PC -BPPV) ,2例为水平半规管性BPPV(HC -BPPV) ,1例为混合型。采用上述两种治疗方法后效果良好。 8例患者经 1～ 7次治疗后症状均缓解。结论 :根据典型临床表现及体位试验可明确诊断 ,根据不同类型BPPV采用不同的复位治疗方法 ,Epley耳石复位法及Lempert翻滚法是治疗BPPV的首选治疗方法。

Objective:To discuss the diagnosis and therapy of benign paroxysmal positional vertigo (BPPV). Method: Clinic data of 8 cases of BPPV are analyzed retrospectively. Medical history of all the cases are collected and neurological checks of ear are taken, and Dixt-Hallpike test and resupine-strophocephaly test are progressed. Epley respositioning procedure and Lempert roll maneuver are separately carried out according to the type of BPPV.Result:Five of 8 cases are PC-BPPV, and 2 of 8 cases are HC-BPPV. The other one is mixed type of PC-BPPV and HC-BPPV. All the cases are remitted via 1-7 times of treatment. Conclusion:The disease might be diagnosed through typical clinical manifestations and posture test, and different repositive treatment might be used according to different type of BPPV. Epley respositioning procedure and Lempert roll maneuver are the preferred therapy to treat BPPV.
