下腔静脉后输尿管的X线诊断(附21例报告)

Radiological Evaluation of Retrocaval Ureter—A Report of 21 Cases

下腔静脉后输尿管为一少见之畸形,系胚胎期下腔静脉发育异常所致。临床多见于成年人,常因为与尿路梗阻有关的症状而被发现。本文搜集21例,均属低袢型。在尿路造影片上可见肾盂及上段输尿管扩张,成反“J”形,输尿管中段内移,可达人体中线处。逆行肾盂造影时输尿管导管弯曲,呈“S”形。CT 可显示输尿管绕至下腔静脉后方。作者认为,本病命名为绕下腔静脉输尿管较为合适。对本病的诊断应以静脉尿路造影及CT 为首选的检查方法。

Retrocaval ureter is a rare abnormality,which is due to abnormal development of inferiorvena cava during gestational age.Usually,the disease is clinically presented in adulthood by the symptomscaused by ureteric obstruction.Twenty-one cases of the low-loop type were reviewed.On IVU the dilatedpelvis and upper ureter forms a reversed“J”shape.And the medial displacement of the middle part of theureter,even to the midline,is also very characteristic.On retrograde pyelography the catheter is curved as“S”shape.By CT scanning the segment of ureter running behind the IVC can be shown.The authors considerthat the proper nomenclature for this disease should be circumcaval ureter.
