小肠原发性肿瘤的X线诊断(附46例报告)

A Radiologic Diagnosis of Primary Tumor of the Small Bowel:Report of 46 Cases.

小肠原发性肿瘤较少见。本文报告46例均经手术、病理证实。通过对临床资料及X 线所见的分析,并复习有关文献,提出了小肠原发性肿瘤的主要X 线表现:(1)肠腔狭窄;(2)肠腔内充盈缺损;(3)管壁僵硬,粘膜破坏,环状皱襞消失;(4)肿瘤性肠梗阻或肠套叠;(5)肿瘤近侧肠管动力性扩张及逆蠕动。并对小肠原发性肿瘤易被漏诊及误诊的原因及肿瘤好发部位等问题进行了讨论。提出了关于小肠良性与恶性肿瘤、肉瘤与癌以及Cohn's 病与小肠癌等的鉴别点。

Primary tumors of the small bowel are relatively rare.The 46 cases have been provedaurgieo-pathologically.Based on clinical and radiologic findings and review of literature,the main x-raymanifestations of primary tumors of the small bowel were:(1)Stenosis;(2)Filling defect;(3)Stiffness ofinvolved intestinal wall with destruction of mucosa and loss of valvulae conniventes;(4)Intestinalobstrution or intussuseption;(5)Dynamic dilatation and reversed peristalsis of the bowel segmentproximal to the tumor.The causes of misdiagnosis,failure of detection and the site of predilection for thetumor were discussed.The differential points between benign and malignant rumors,sarcoma andcarcinoma,Crohn's disease and carcinoma were mentioned.
