老年麻醉有关问题经尿道前列腺切除手术体液平衡的探讨

The Fluid Balance in TURP Patients

本文对14例TURP手术病人术前、术毕及术后2、4h进行血浆电解质及渗透浓度的测定。以改良Ba11ey法测定血液内冲洗液的吸收量。结果表明在采用“低压冲洗”这一技术下,术毕、术后2、4h血浆电解质及渗透浓度虽均较术前略降低,但无统计学意义(P>0.05)。冲洗液回吸收量亦以术毕时最高29.07±39.65ml,术后2、4h逐渐减少。值得注意的是,术中4例出现一过性心律失常,我们认为与原有的疾病有关,因此术中有必要行心电监测。

The serum K,Na,Cl,and osmolality were measured in 14 patients,before operation,at the end of operation and 2,4 hours postoperatively. The results showed that the elyctrolytes and osmolality were decreased postopera- tively but of no significant difference (P>0.05) as compared with the control values. We still calculated the absorbed volume of irrigating fluid through measuring the manitol concentration in blood with modified Bailey method. The values showed that it was 29.07±39.65 ml at the end of operation and decreased gradually at 2,4 hours postoperatively. Dysrhythmia as noted on EKG monitoring was shown in 4 patients. This was presumably due to the accompanied disease. We used the suprapubic trocar for these patients since it decrease the bladder pressure, may be it is the key point to get the values from our experiments.
