颈动脉体瘤的外科治疗

Surgical treatment of carotid body tumor

目的 探讨颈动脉体瘤手术处理和主要并发症的防治原则。方法 选择我院 1979年～ 2 0 0 3年经手术治疗的颈动脉体瘤患者 33例 (其中男 12例 ,女 2 1例。年龄 19～ 6 4岁 ,平均 35 .7岁 ,全部病例均经病理证实为颈动脉体瘤 ) ,对其治疗过程进行总结与分析。结果 33例病人行颈动脉解剖、肿瘤剥除 2 6例 ,行颈动脉体瘤及相应区段颈动脉切除、颈动脉结扎 7例。 33例病人 1例死亡 ,其余病人痊愈出院。结论 术前选择性血管造影、确实可靠的颈动脉压迫训练、选择适当的手术方法及术后严密观察及时处理并发症 ,是颈动脉体瘤外科治疗成功的关键。

Objective To study the surgeryand complication prevention of carotid body tumor.Methods Thirty-three patients whose ages were from 19 to 64 years old,were included in this study.Histological diagnosis was carotid body tumor for all patients.Treatment process was summarized and analyzed.Results Thirty-three cases except 1 were recovered from illness.Tumor was detached from carotid artery for 26 patients;artery resection was done for 7 patients.Conclusion Selective carotid arteriography,reliable Matas training,proper operation,close observation postoperatively and complication treatment in time are key factors for carotid tumor surgical treatment.
