永存动脉干长期误诊为法乐氏四联症2例报告并文献复习

Persistent Truncus Arteriosus(PTA) Was Misdiagnosed as Tetralogy of Fallot(TOF):Report of 2 Cases and Literature Review

目的 :提高对永存动脉干的病理基础、临床和影像学表现的认识。方法 :报告 2例成人型永存动脉干的临床及各种影像学表现并做相关文献复习 ,阐述了永存动脉干的病理分型和相应的血管造影、彩色多普勒的改变。结果 :结合超声心动图及血管造影所见并按 Van Praagh的四型分类法 , 型及 型各 1例 ,均为右弓右降和四瓣叶 ,病理及生理多变 ,临床表现不一。结论 :永存动脉干的诊断、分型以及与其它多种先天性心血管畸形的鉴别可首选超生心动图 ,再由造影重点检查 ,不仅有益于病理分型 ,还有益于并存畸形的诊断。

Objective:To improve the understanding of Persistent Truncus Art eriosus(PTA) on clinical and imaging findings.Methods:The findings of X ray film, echocardiography and cardiovascular angiography in two cases of PTA were reported and the relative literature was reviewed.Results:Of the findings of echoca rdiography and cardiovascular angiography in two cases, one was Ⅱand another was Ⅲ by the category of PTA of Van Praagh.PTA had a various clinical appearances and variable imaging findings.Conclusions:Application of multiple examinations especially with echocardiography and cardiovascular angiogrophy had important values in diagnosis of PTA.
