原发性高血压患者的个性、情绪因素的研究

The Study of Personality and Emotion in Patients with Essential Hypertension

目的 :研究个性、情绪因素与原发性高血压症状的关系 ,探讨原发性高血压患者使用抗抑郁焦虑剂治疗后的血压变化。方法 :以 30名正常人为对照 ,研究 30名原发性高血压患者的MMPI、LES、SS、HAMA、HAMD ,并将患者随机分为苯那普利治疗组 (A组 )和苯那普利加用帕罗西汀治疗组 (B组 ) ,在服药第 2 8天末比较A、B两组患者的血压变化、HAMA及HAMD得分变化。结果 :患者组的HAMA、HAMD得分高于正常对照组 ,患者组的MMPI得分显示在诈病分数 (F)、多疑病 (Hs)、抑郁 (D)、精神病态 (Pd)、神经衰弱 (Pt)、轻躁狂 (Ma)量表分值均高于 70分。B组患者在服药 2 8天后血压下降较A组明显。HAMA得分与Ma呈正相关 ,HAMD得分与D呈正相关。血压变化与HAMA及HAMD得分变化呈正相关。结论 :原发性高血压患者的社会心理因素中重要的是患者的个性特征 ;焦虑抑郁等负性情绪普遍存在于高血压患者中 ;抗抑郁抗焦虑治疗对原发性高血压的治疗有协同作用。

Objective:To investigate the relationships between personality、emotion and essential hypertension,and to observe the change of blood pressure and emotion after being given antidepressants.Methods:A series of scales were used to assess 30 patients with essential hypertension and 30 controls,respectively.The EH patients were divided into group A and B.Group A was given Benazepril,and B was given Benazepril and Paroxetine.Results:Patients with EH had higher scores in the scales of HAMA and HAMD compared with that of the control(P<0.05).The scores of the EH patients in the scales F、Hs、D、Pd、Pt and Ma of MMPI were more than 70.There was no significant difference in the scales LES and SS.The levels of blood pressure both decreased in the groups A and B,but the level of group B was lower than that of group A(P<0.05).The scale of HAMA exhibited positive correlation with the scale of Ma,so did the HAMD and D scales.The reduced value of blood pressure exhibited positive correlation with the scales of HAMA and HAMD.Conclusion:Among the psychosocial factors of EH,the most important ones was the patients' inner personalities and negative emotions such as anxiety and depression.Paroxetine could help to reduce the blood pressure in EH.
