鼻咽癌上皮性标志表达的异质性及其组型研究

STUDY ON THE HETEROGENEITY AND PATTERNSOF EPITHELIAL MARKER EXPRESSION IN NASOPHARYNGEAL CARCINOMA (NPC)

采用免疫组化方法,应用一组抗癌胚抗原(CEA)、上皮膜抗原(EMA)和不同分子量细胞角蛋白抗体,对123例不同组织学类型鼻咽癌上皮性标志的表达进行了研究。结果表明,鼻咽癌上皮性标志的表达既存在明显异质性,也存在一定规律性。鼻咽癌上皮性标志和细胞角蛋白的表达均可分别归纳为4种表达组型。这些表达组型与鼻咽癌组织学类型密切相关。一定的组织学类型总是显示一定相应的上皮性标志和细胞角蛋白的表达组型。在鼻咽癌的角化性鳞癌与腺癌、角化性鳞癌与非角化性癌、非角化性癌与鼻咽型未分化癌的鉴别中,抗细胞角蛋白单抗的应用是有实用价值的。

The epithelial marker expression of 123 NPC cases was studied immunohistochemic-ally using a panel of antibodies against CEA, EMA and a variety of different molecular weight cytokeratins. The results indicated that there were obvious heterogeneity and regularity in epithelial marker expression of NPC. Numerous expression combinations could be detected in these 123 cases, but, however, they could be attributed to 4 patterns of epithelial marker expression and of cytokeratin expression, respectively. The expression patterns were closely correlated with the histological types of NPC. A given histological type always showed certain equivalent cytokeratin expression pattern(s). The application of monoclonal anticytokeratin antibodies has practical significance in assistance to differentiating the histological types of NPC, namely, keratinizing squamous cell carcinoma (KS-CC) vs. adenocarcinoma (AC), KSCC vs. nonkeratinizing carcinoma (NKC), and NKC vs. undifferentiated carcinoma of nasopharyngeal type (UCNP).
