PCR快速检测病毒性脑炎患者脑脊液中HSV、CXV和CMV

Rapid Diagnosis of Herpes Simplex Virus,Coxsackie Virus and Cytomegalovirus in the Cerebrospinal Fluid in Patients with Viral Encephalitis by Polymerase Chain Reaction

本文应用聚合酶链反应（ＰＣＲ）和逆转录ＰＣＲ（ＲＴ－ＰＣＲ）技术检测了２７例临床诊断为病毒性脑炎患者和２５例其他神经疾病（ＯＮＤ）对照组患者脑脊液（ＣＳＦ）中单纯疱疹病毒（ＨＳＶ）、柯萨奇病毒（ＣＸＶ）和巨细胞病毒（ＣＭＶ）。２７例患者组检出ＨＳＶ１３例、ＣＸＶ２例、ＣＭＶ１例，总阳性率为５９．３％，而ＯＮＤ对照组皆为阴性。随着检测技术的完善及对更多种类病毒采用ＰＣＲ早期快速诊断方法的建立，将有效地指导特异性抗病毒药物的选择应用，从而提高治愈率，降低病死率和改进预后。

The aim of this study was to determine the applicative values of polymerase chain reaction(PCR)and reverse transcription polymerase chain reaction (RTPCR)in diagnosis of viral encephalitis. 3 pairs of primers were selected in the highly conserved part of herpes simplex virus(HSV),Coxsackie virus(CXV)and cytomegalovirus(CMV)genome,Cerebrospinal fluid(CSF) from 27 patients with clinically definite viral encephalitis and 25 control patients with other neurological diseases(OND)were involved in this study. HSV,CXV and CMV positive CSF specimens were demonstrated in 13, 2 and 1 patients of these 27 patients with viral encephalitis,respectively. The total positive rate was 59.3%while all the specimens from patients with OND showed negative. The study also revealed a consistent correlation between the amount of HSV-DNA in the CSF detected by PCR and the severety of clinical manifestation in patients with herpes simplex virus encephalitis(HSE).With the improvement of PCR、RT-PCR techniques and the constitution of PCR procedures for widely detective spectrum of viruses,it will be expected that the principles for choice of specific antiviral drugs based on the results of PCR detection will be beneficial to recovery and outcome of patients with viral encepbalitis.
