粘附分子在分娩期宫颈扩张中的子宫下段肌层血管内皮细胞上的表达

摘要 目的 :观察粘附分子在分娩期宫颈不同扩张程度下的子宫下段肌层组织中血管内皮细胞上的表达 ,探讨其在宫颈扩张机制中的作用。方法 :用免疫组织化学SP法 ,检测E 选择素、血管细胞粘附分子 1(VCAM 1)、细胞间粘附分子 1(ICAM 1)在宫颈不同扩张程度下子宫下段肌层血管内皮细胞上的表达。结果 :随着宫颈的扩张 ,E 选择素和VCAM 1在子宫下段血管内皮细胞上表达明显增加 ,在宫颈开大 >6cm组的表达与宫颈口开大 <2cm组的表达差异有显著性或非常显著性 (P <0 .0 5 ,P <0 .0 1)。ICAM 1在各组的表达差异均无显著性。结论 :E 选择素和VCAM 1在宫颈扩张过程中表达增加 ,是宫颈扩张过程中的一个重要环节。 Objective: To determine the expression of E-selectin?VCAM-1, and ICAM-1 in the lower uterine segment during parturition and to investigate correlations with the stage of cervical dilatation. Methods: Biopsy specimens were taken from the lower uterine segment of 40 women undergoing cesarean section at different stages of cervical dilatation (<2 cm, n = 10; 2~3 cm, n = 10; 4~6 cm, n =10;> cm, n =10). The expression of E-selectin?VCAM-1, and ICAM-1 was detected by immunohistochemistry. Results: The expression of E-selectin and VCAM-1 was increased with the dilation of cervix, which significantly higher at > 6 cm dilatation than at < 2 cm dilatation ( P <0.05, P <0.01). There was no difference in expression of ICAM-1 in each stage of cervical dilation. Conclusions: Cervical dilatation is associated with up-regulation of adhesion molecules (E-selectin, VCAM-1) in the lower uterine segment that is a key role in the process of cervical dilation.
