脑室内不典型肿瘤及少见肿瘤的MRI误诊分析

The misdiagnostic analysis of MR imaging features about atypical or rare intraventricular tumors

目的对脑室肿瘤的不典型MR征象和一些发生在脑室内的少见肿瘤共15例进行回顾性分析,以提高对其诊断和鉴别诊断的能力。方法我院近5年来经手术和病理证实的脑室内肿瘤43例,其中误诊和少见病例共15例,分属于8类病种。回顾性分析这15个病例的MR影像表现并结合文献进行讨论。结果这些病例有中央性神经细胞瘤、髓母细胞瘤各3例,少枝胶质瘤、脉络丛乳头状瘤、室管膜瘤各2例,脑膜瘤、星形细胞瘤、海绵状血管瘤各1例。12例诊断错误。结论对脑室肿瘤的诊断除要掌握其典型征象外,还要加强对少见病或少见征象的认识,才能提高诊断正确率。

Objective To make correct diagnosis of intraventricular tumors, 15 patients of rare intraventricular tumors or with uncommon magnetic resonance (MR) imaging features were analysed retrospectively. and Methods Fifteen patients of 43 intraventricular tumors that arising from or extending into ventricle, which belonged to 8 categories and were confirmed by operation and pathology between 1997 June and 2002 July,were misdiagnosed or rare tumors. MR imaging features of these tumors were analysed retrospectively and discussed with literatures.Results The 15 cases included central neurocytomas (3cases), medulloblastomas (3cases), oligodendrogliomas (2cases), choroids plexus papillomas( 2cases), ependymmomas (2cases) and 1 case of meningiomas, astrocytoma and cavernoma respectively. 12 cases of them were misdiagnosed.Conclusion In order to make correct diagnosis of intraventricular tumors,both characteristic imaging features and awareness of these uncommon tumors or atypical features are in great need.
