摘要
目的 探讨肾细胞癌CT表现在动态增强CT扫描中随时间推移变化过程。方法 18例肾细胞癌患者行病灶靶平面同层动态CT扫描。由二位副高职称以上腹部影像专业医师共同评价肾细胞癌出现的征象及其与时间窗的相关性。结果 肾细胞癌CT征象包括 :瘤区异常血管、肿瘤实质显著性强化、RCC与肾实质交界区形态。肿瘤CT征象显示与时间窗的相关性 :①异常肿瘤血管出现 ,显示最佳及消退时间平均分别为 2 3 .9s ,3 8.2s ,46.3s ;②肿瘤实质强化出现最早时间为 2 8.9s ,最佳时间为显示征象最丰富层面的时间 ,即 70 .2s ;③低密度包膜的演变与肿瘤实质一致。结论 靶平面充分显示了肿瘤的关键的CT征象 。
Objective To investigate the CT features and their evolution of renal cell carcinoma (RCC) along with the changing of time window by using contrast enhanced single-level dynamic spiral CT. Methods Eighteen cases of RCC underwent single-level dynamic spiral CT (SCT) scan and routine CT scan of the kidney. The imaging reviewers consisted of two experienced radiologists, who reviewed the various possible CT features of RCC obtained under time window, especially the dynamic CT changes of RCC correlated to the evolution of the time. All the CT manifestations were compared with pathological observations. Results CT manifestations of RCC at time window included as follows: ① neovascularity with tumor masses. ② remarkable enhancement of tumor masses. ③ clear or blurred demarcation of the interface between tumor masses and neighbor renal parenchyma border. Dynamic evolution of tumors CT findings: ① The time for the emergence depiction and resolution were at 23.9, 38.3, 46.3 seconds, respectively.② The earliest time for tumor mass to show enhancement was at 28.9 seconds, and the time to depict the richest CT features of tumors was at 70.2 seconds. ③ The evolution of hypodense pseudocapsule of RCC was similar to that of the tumor mass. Conclusion Under temporary window, the target slice can fully depict the possible spectrum of RCC CT manifestations, which reflect the underlying pathology of the tumor.The remarkable enhancement is the most frequent SCT features of RCC.
