江苏省恶性肿瘤高低发地区肿瘤死因分析

An Analysis on Death Cause of Cancer in High and Low Incidence Areas of Jiangsu Province

[目的]探讨江苏省内恶性肿瘤高发区和低发区主要恶性肿瘤死亡分布的差异.[方法]选择高发区大丰市和低发区赣榆县1996～2002年恶性肿瘤死亡资料,对死亡率、去肿瘤期望寿命、时间趋势等作对比分析.[结果]尽管顺位有所不同,但两地居民肿瘤死因前4位均为肝癌、胃癌、食管癌和肺癌.除男性食管癌死亡率在高低发区没有显著差异外,男性中其它3种肿瘤及女性4种肿瘤的粗死亡率和标化死亡率高发区均明显高于低发区.两地食管癌和肺癌平均死亡年龄基本接近,而高发区胃癌和肝癌平均死亡年龄明显低于低发区.7年来两地肺癌死亡率均呈明显上升趋势.4种恶性肿瘤对居民期望寿命的影响高发区和低发区为分别3.13岁和1.91岁.[结论]两地前4位恶性肿瘤死亡率及构成存在明显差异,并表现为不同的流行病学特点,有必要开展研究探讨造成差异的原因.

To investigate the difference in distribution of the leading death cause of cancer in high and low incidence areas of Jiangsu province.Death registry data in high incidence area Dafeng county and low incidence area Ganyu county were used to explore mortality of major cancers,cause-specific deductible life and mortality trend.The top four cancers were liver,esophageal,stomach and lung cancer in both high and low incidence areas with different order.The crude and adjusted mortality of all cancers were significantly higher in high incidence area than those in low incidence area except for the esophageal cancer in male.The average death age for the esophageal and lung cancers was comparable between two areas,however mean death age of patients with stomach and liver cancers was significantly younger in high incidence area than that in low incidence area.The mortality of lung cancer had been progressively increasing in both areas from 1999 to 2002.Deducted life time by the top four cancers compared with excepted life time was 3.13 years and 1.91 years in the high and low incidence areas respectively.[Conclusions]There is a marked difference in cancer mortality of top 4 cancers in high and low incidence areas in Jiangsu with different epidemiologic features.It is necessary to explore the different cause with cancer in two areas.
