COPD患者机械通气营养支持的比较研究

COMPARISON OF MUTRITION SUPPORT IN MECHANICAL VENTILATED CHRONIC OBSTRUCTIVE PULMONARY DISEASE PATIENTS

目的 :评价营养支持对接受机械通气的慢性阻塞性肺病 (COPD)患者的临床作用。方法 :采用随机、对照的实验设计。 33例患者分肠内营养 (EN)组 (1 1例 ) ,肠外营养 (PN)组 (1 1例 )和普通组 (1 1例 )。EN和 PN组患者接受等氮等热量营养支持 ,普通组仅给予常规输液与普通饮食。结果 :EN组氮平衡、前白蛋白浓度高于 PN和普通组 ,组间差异有统计学意义。肺部感染治愈率 EN和 PN组结果相似 ,优于普通组。 EN组成功撤机率高于其它组。 EN组营养费用低于 PN组 ,但高于普通组。结论 :综合临床效能 。

Objective:to evaluate the effects of nutrition support in the chronic obstructive pulmonary disease patients(COPD)who were undergoing the mechanic ventilation.Methods:33 cases of mechanical ventilated patients were randomized into enteral nutrition (EN) group(n=11), parenteral nutrition (PN) group (n=11) and the ordinary group (n=11), EN and PN groups were given the same energy and the same nitrogen,the ordinary group was given the routine intravenous therapy or ordinary diet.Result:Nitrogen balance and pro albumin concentration in EN group was improved,and the difference was significant. The lung infection rates were similar to that fo the former two groups .The nutrition cost in EN group was lower than in PN group but higher than in the ordinary group.Conclusion:As far as the clinical effects were concerned,enteral nutrition was a better nutrition support approach for the patients who were undergoing ventilator therapy.
