摘要
目的 :探讨经皮肝穿刺病理活检在肝移植术后并发症诊断及鉴别诊断中的意义。方法 :对 9例 1 0次肝移植术后 35次肝穿刺活检病理结果结合临床进行分析。结果 :35次肝穿刺病理结果中 ,2 2次 (6 2 .8% )呈阳性 ,包括急性排斥反应、CMV感染、HBV感染、药物性肝损害、肝瘀胆等。 1例患者肝穿刺术后出现腹腔内大出血须行剖腹手术。结论 :肝穿刺活检对肝移植术后排斥反应的早期诊断及鉴别诊断具有决定性意义 ,在术后 1周内应进行肝穿刺活检以早期诊断或除外排斥反应 。
Objective:To explore the clinical significance of liver biopsy in diagnosis and differentiated diagnosis for complication after liver transplantation.Methods:The liver biopsy pathology combine with clinical manifestation in 10 consecutive liver transplantations were analysed.Result:Total 35 episodes of liver biopsy were performed after liver liver transplantation , abnormal findings (positive) were observed in 22 (62 8%) episodes of liver biopsy pathology . The positive outcomes proved to be acute or chronic rejection, viral hepatitis, medical liver lesion and cholestasis. 1 case of patient had bleeding intraperioneal cavity after liver biopsy and needed laparotomy.Conclusion:Liver biopsy pathology is crucial in detection of early rejection and differentiating the complication after liver transplantation ; The complication of liver biopsy should be avoided in the procedure.
