经皮椎体成形术治疗骨质疏松性椎体压缩骨折 被引量:1

摘要 目的 探讨经皮椎体成形术 (percutaneousvertebroplasty ,PVP)治疗不同压缩程度骨质疏松性椎体压缩骨折的操作技术与临床效果。方法 对 2 8例 (31椎 )骨质疏松性椎体压缩骨折 ,根据椎体压缩程度分为Ⅰ ,Ⅱ ,Ⅲ度 ,年龄 5 6~ 82岁 ,平均 6 9岁 ;伤椎分布在T11~L3 椎体 ,2 0个胸椎 ,11个腰椎。所有伤椎均在C -臂透视下行单侧椎弓根PVP治疗。手术前后疼痛程度用直观模拟量表 (visualanaloguescales,VAS)记录 ;术后临床效果采用Stauffer-Coventry(SC)评价。对于骨折的程度与骨水泥用量、VAS及SC进行记录 ,分别采用配对t检验及Kruskal-Wallis检验进行分析。结果 所有病例获得随访 ,术后随访 6~ 2 4个月 ,平均 18个月。 31椎均穿刺成功 ,Ⅰ度压缩骨折骨水泥用量 (4 7± 0 5 )ml,Ⅱ度 (3 5± 0 6 )ml,Ⅲ度 (2 2± 0 4 )ml;术后第 2天VAS比术前有明显下降 ,Ⅰ度压缩骨折较术前下降 3 5~ 6 0 (t=13 2 3,P <0 0 0 1) ,Ⅱ度 3 4~ 6 2 (t=18 2 2 ,P <0 0 0 1) ,Ⅲ度 3 5~ 6 0 (t=2 0 4 7,P <0 0 0 1) ;术后临床效果经Kruskal-Wallis检验 ,3组之间差异无显著性 (χ2 =0 10 0 ,v =2 ,P =0 95 1)。结论 PVP治疗骨质疏松性椎体压缩骨折安全有效 。
