摘要
目的探讨喉癌组织中基质金属蛋白酶(MMP-2,MMP-9)、细胞黏附因子44变异体6(CD44v6)、增殖细胞核抗原(PCNA)、血管内皮生长因子(VEGF)、组织蛋白酶-D(Cath-D)、E-钙黏素(E-cadherin)、p53和 nm23蛋白的表达等因素与喉癌侵袭转移的关系及在预测淋巴结转移中的价值。方法用免疫组化方法检测70例喉癌标本中 MMP-2、MMP-9、CD44v6、PCNA、VEGF、Cath-D、E-cadherin、p35和 nm23等的表达。采用 Logistic 逐步回归的方法对11项与喉癌淋巴结转移有关的指标进行筛选。结果喉癌中 MMP-9表达阳性者,发生淋巴结转移的机率最高;nm23、E-cadherin 表达阴性或低表达者发生淋巴结转移的机率高。喉癌的淋巴结转移还与原发部位、组织学分级有关。结论喉癌组织中 MMP-9、E-cadherin 和 nm23的表达情况,可作为术前预测患者淋巴结转移的指标。
Purpose To study the function of MMP-2、MMP-9、CD44v6,PCNA,VEGF,Cath-D,E-cadherin p53 and nm23 in the invasion and metastasis of laryngeal cancer and their value in estimating early lymph node metastasis.Methods The expressions of MMP-2,MMP-9,CD44v6,PCNA,VEGF,Cath-D,E-cadherin,p53 and nm23 were examined by immunohistochemical technique in 70 patients with laryngeal cancer.Logistic stepwise regulation model was used to analyze the influence of 11 clinicopathologic factors on the lymph node metastasis.Results With the established equation to estimate lymph node metastasis,the frequency of lymph node metastasis was the highest in patients with positive MMP-9 expression, and the lower the E-cadherin expression,the higher the frequency of lymph node metastasis.The expression of nm23 in patients with lymph node metastases was significantly decreased in comparision with patients without metastases.The frequency of lymph ndoe metastasis was also related with primary location and histological grade.Conclusion Logistic regression analysis showed that the expression of MMP-9、E-cadherin and nm23 were highly useful for the preoperative diagnosis of lymph node metastasis in laryngeal cancer.
