食管癌综合治疗的临床疗效分析

Observe the clinical results of esophagus carcinoma combination therapy

目的:分析化疗、放疗、化疗即三明治法综合治疗食管癌的临床疗效并与单纯放疗进行比较.方法:1995年4月至1997年10月对食管癌综合治疗与单纯放疗进行随机分组研究.两组病例均为38例.两组放疗方法和剂量相同,每次1.8～2.0Gy,每周五次,总量达DT65Gy左右/6.5～7周.综合组所用化疗方案DDP、5-FU、PYN,放疗前、后一周各行一疗程化疗.结果:综合组临床疗效CR39.5%(15/38),PR 50%(19/38),NC 5.3%(2/38)和PD 5.3%(2/38),有效率(CR+PR)89.5%(34/38).单放组CR13.2%(5/38),PR 36.8%(14/38),NC 36.8%(14/38)和PD 13.2%(5/38),有效率(CR+PR)50%(19/38).综合组的CR及有效率均优于单放组.(P<0.05).结论:对食管癌进行综合治疗能提高临床疗效.

Purpose: Observe the clinical results of esophaqus carcinoma combination therapy and compare it with the sole ra-diotherapy's. Methods: From April 1995 to Octomber 1997 we do random seperately research to the esophaqus carcinoma combi-nation therapy and sole radiotherapy. The totle number of the cases is 38. The method and dose of the two radiotherapy is the same. It's 1. 8~2. 0 Gy per time,five times a week. And the totle dose reach 65Gy/6. 5~7w. The chemotherapy regimen is DDP, 5-Fu,PYM and get one time before radiotherapy and the other time after radiotherapy. Resuits: Combination therapy line's clini-cal results is CR 39. 5% ,PR 50% ,NC 5. 3% ,PD 5. 3%,CR + PR 89. 5%. The sole radiotherapy line's clinicl results is CR 13. 2% ,PR 36. 8% ,NC 36. 8% ,PD 13. 2% ,CR + PR50%. The combination therapy's CR and effective rate are all better than the sole radiotherapy's(P<0. 05). Concl usion: Combination therapy can enhance the clinical effect of Esophaqus carcinoma.
