显微手术治疗脑胶质瘤的临床分析

The clinical study of microsurgical resection of malignant glioma

①目的 探讨显微手术切除脑胶质瘤的疗效及其技巧。②方法 回顾性分析显微手术切除及术后病理确诊的胶质瘤22例患者的临床资料。③结果 手术全切14例,次全切6例,大部分切除2例。术后运动及语言等神经功能未受影响,无手术并发症。④结论 脑胶质瘤采用显微手术效果好,是手术治疗的首选。尤其对于位于重要功能区的肿瘤,只要严格按肿瘤边界切除肿瘤,即可减少功能障碍的发生,提高术后生存质量。

Objective To study and evaluate the application of the microsurgical resection in the patient with gli-oma. The surgical skills were studied. Methods Harboring in different region of hemisphere, 22 cases of malignant gliomas, including 3 cases of pilocytic astrocytoma ,7 cases of fibrillary astrocytoma ,4 cases of anaplastic astrocytoma ,5 cases of glioblastoma multiforme and 3 case of oligodendrogliomas, were studied. 7 cases of gliomas in eloquent areas. The diameters of tumors were from 2.5cm to 6. 8cm , with mean size at 5.2cm, The technique based on identification and dissection of the sulcus adjacent to the tumour followed by en bloc tumour resection was used. The patients'postoperative condition were reviewed. Results Radical resection was done in 14cases, subtotal resection in 6 cases , partial resection in 2 cases. The clinic status of all patient showed improvement, and the critical functions were not significantly affected. There was no complication due to the resection. Conclusion The microsurgical resection is relatively safe and reliable by using en bloc tumour resection. The ability to obtain a complete resection is associated with a better prognosis.
