摘要
目的 :研究骨性关节炎骨骼质量变化特点 ,探讨骨性关节炎与骨密度变化的关系。方法 :用法国DMS公司生产的UBIS 30 0 0超声骨密度仪测定 10 6例骨性关节炎 (OA组 )和 92例正常体健者 (对照组 )的超声振幅衰减(broadbandultrasoundattenuation ,BUA)、超声波传导速度 (speedofsound ,SOS)值、T值 (T score)及Z值 (Z score) ,并对比分析。结果 :OA组的BUA、SOS、T值、Z值均明显低于对照组 ;女性BUA明显低于男性 ,OA组女性SOS亦低于男性 ,而另 2项指标男女性别间无统计学意义。结论 :不同性别之间骨密度和骨结构存在差异 ,女性较男性更易患骨质疏松 ,骨性关节炎患者存在骨密度和骨弹性明显下降 ,伴随骨结构变化 ,表明骨性关节炎的发生与骨质疏松有密切关系。定量超声骨密度检测对早期诊断骨性关节炎合并的骨质疏松有重要意义。
Objective: To investigate the characteristic of changes of bone in osteoarthritis, and detect the relationship between bone status and osteoarthritis. Methods: One hundred and six patients with osteoarthritis and 92 healthy person as control were involved in this study. Quantitative ultrasound was performed to determine bone mineral density in the heel by UBIS 3000 (DMS, France), and BUA, SOS, T score and Z score were statistically analyzed. Results: Values of BUA, SOS, T score and Z score were significantly lower in patients with osteoarthritis than those in control. As compared with male, marked decrease in BUA was found in female from both groups, as well as a decrease in SOS from osteoarthritic female. No sex difference was found in T score and Z score. Conclusion: Sex difference was in bone mineral density and bone structure. Female was more liable to osteoporosis. A decrease of bone elasticity and a reduction in bone mineral density were found in patients with osteoarthritis, associated with changes in bone structure. It revealed that there was a close association between osteoarthritis and osteoporosis. Quantitative ultrasound is of a significance in the early diagnosis of osteoporosis in patients with osteoarthritis.
