缺血性脑血管病颅内动脉狭窄或闭塞的TCD、MRA对比研究 被引量:9

A study of TCD compared with MRA in the diagnosis of occlusive and stenotic disorders of intracranial arteries in ischemiic cerebral vascular disease

摘要 目的 比较经颅多普勒 (TCD)与磁共振血管成像 (MRA)对缺血性脑血管病颅内动脉狭窄或闭塞诊断的准确性。方法 回顾分析 5 0例缺血性脑血管病患者的TCD与MRA检测结果 ,全部病例均有脑血管造影 (DSA)对比验证。结果 TCD对缺血性脑血管病颅内动脉狭窄或闭塞诊断的特异性为 98.6 % ,敏感性为 83.9% ;MRA分别为 92 .6 %和 79.7%。TCD假阳性率为 10 .3% ,假阴性率为 2 .5 % ;MRA分别为 35 .7%和 3.5 %。结论 在诊断缺血性脑血管病颅内动脉狭窄或闭塞方面 ,TCD与MRA均有很好的可靠性 ,但TCD较MRA有更高的准确性。 Objective To evaluate the accuracy of TCD and MRA in the diagnosis of occlusive and stenotic disorders of intracranial arteries.Methods 50 cases with ischmic cerebral vasculiar disease were collected.All the patients underwent TCD,MRA and DSA.All the examination results were compared.Results The specificity for detecting the stenosis and occlusion of intracranial arteries was 96.7% for TCD and 92.4% for MRA.The sensitivity was 83.9% for TCD and 79.7% for MRA.The false positive rate was 10.3% for TCD and 35.7% for MRA.The false negative rate was 2.5% for TCD and 3.5% for MRA.Conclusions Both TCD and MRA are relatively quite reliable in detecting stenosis and occlusion in the intracranial artery,but TCD is more accurate than MRA.
