产后大出血急诊栓塞治疗的临床价值——附16例报告

Clinical Value of Emergency Embolization in the Treatment of Postpartum Hemorrhage (Report of 16 Cases)

目的:探讨急诊双侧髂内动脉栓塞术(IIAE)治疗产后大出血的临床价值和意义。方法:16例患者中,自然生产后大出血13例,剖腹产后大出血2例,引产后大出血1例。急诊选择性血管造影确诊后行双侧髂内动脉栓塞,栓塞剂为明胶海绵。结果:所有病例栓塞后15 min内出血停止,取出宫腔内填塞纱布条,挤压小腹,证实无出血后结束手术。结论:急诊栓塞治疗产后大出血是一种有效、迅速、安全的止血方法。

Objective:To investigate the clinical value of emergency bilateral internal iliac arreries embolization( IIAE) in the treatment of postpartum hemorrhage. Methods; 16 patients with postpartum hemorrhage including 13 cases of spontaneous postpartum, 2 cases of postcesarean section bleedin,1 case of induced abortion bleeding were treated with bilateral internal iliac arteries embolization using fresh gelfoam after angiographic diagnosis. Results; All of the 16 cases with postpartum bleeding were successfully controlled within 15 minutes. No rebleeding was confirmed after embolization. Conclusion; Emergency embolization is an effective, quick and safe therapeutic measure for treatment of massive postpartum hemorrhage.
