壁层腹膜修复胆管缺损模型组织中转化生长因子-β1的表达

Implication of transforming growth factor-β1 expression in the repaired bile duct defect with free parietal peritoneal graft

目的 :观察壁层腹膜修复胆管组织中转化生长因子 -β1(TGF-β1)的表达情况 ,探讨壁腹膜修复胆管缺损可行性 ,为临床上胆管缺损的修复提供理想材料。方法 :(1) 2 4条健康杂种犬 ,经开腹手术切除部分胆管管壁 ,建立胆管缺损模型。切取较缺损稍大的一块壁层腹膜 ,浆膜面对胆管腔 ,间断缝合于胆管缺损边缘 ,外用大网膜覆盖并固定。 (2 )术后 4周、8周、12周、2 8周、5 2周分别取 4条实验犬 ,切取修复段胆管行免疫组化检查 ,观察 TGF-β1的表达情况。结果 :TGF-β1在术后 12周前表达明显增高 ,2 8周后 ,均明显下降 ,与术前相比差异无显著性 (P>0 .0 5 )。结论 :壁层腹膜修复胆管缺损后期 ,TGF-β1低表达 ,提示无形成胆管狭窄趋势。

Objective:To observe the expression of transforming growth factor-β1(TGF-β1) expression in the repaired bile duct defect with free parietal peritoneal graft and explore the feasibility of using autogenous free peritoneal graft for the repair of bile duct defect, and desire to find ideal material for the clinical management of bile duct injury.Methods:(1)24 local healthy mongrel dogs underwent right rectus abdominis laparotomy and had their bile duct resected to establish bile duct injury models. The parietal peritoneal flap was harvested near the abdominal incision with the similar dimension of bile duct defect, and was sutured to the edges of the excised bile duct with a coverage of omentum outside. (2)Laparotomy was performed tissues of common bile duct, especially the repaired segment, was harvested, and the expression of TGF-β1 was studied by using immunohistochemical SP staining method.Results:TGF-β1 was expressed strongly before 12 weeks, but after 28 weeks, TGF-β1 were expressed weakly, which had no significant difference from preoperation(P>0.05).Conclusion:Free autogenous peritoneum can be used for regeneration of bile duct tissue in a canine model because of the low expression of TGF-β1,which indicates impossibility of biliary stricture.
