This text introduces a fire & explosion accident initiated by valve maloperation in one petroleum and petrochemical plant, analyses the causation of the accident and the malopera-tion, and proposes to strictly abide by the Qualification Certificate system, to pull off the maloperation prevention measures of System Intrinsic Safety.
