摘要
目的以左心房构型超声新表达指标评价左心室舒张功能的改变。方法用二维超声于左心室收缩末期测量左心房最大内径(LAD)、面积(LAA)和容积(LAV),用体表面积标化出左心房内径指数(LADi)、面积指数(LAAi)和容积指数(LAVi),观察各危险层高血压患者LADi、LAAi、LAVi3项指标的变化,同时测量二尖瓣血流参数E/A比值。结果高血压患者LADi、LAAi、LAVi均较正常组增大,差异具有显著性(P<0.05)。并随危险度分层的增高,差异愈显著。不同危险层之间比较差异具有显著性(P<0.05)。而E/A比值仅在中危组以上与正常组比较差异具有显著性。结论左心房构型超声新表达指标评价左心室舒张功能优于E/A比值。
Objective To evaluate the left ventricular diastolic function in hypertension with a new echocardiographic expression index of left atrial geometry. Methods The diameter,area and volume of the left atrium and the mitral blood inflow parameter E/A ratio were measured in patients with different risk levels at the end-systolic phase of left ventriculus by two dimensional ultrasoud. The index of the diameter(LADi), area (LAAi) and volume (LAVi) in relation to body surface area were calculated. Results The LADi,LAAi and LAVi in hypertension group were higher than that in control group with sigificant difference (P<0.05),and the difference was more obvious as the elevating of risk level in hypertensive patients.The difference was significant(P<0.05) between different risk level hypertensive patients.But the significance difference was only found in E/A between middle(or higher) risk level hypertensive patients and normal subjects. Conclusion The novel echocardioographic expressions index in left atrial geometry are more sensitive than E/A ratio for evaluating left ventricular diastolic function.
