黄芪注射液治疗糖尿病肾病的疗效观察

Investigation of the effect of Astragalus injection on treating diabetic nephropathy

目的观察黄芪注射液对临床期糖尿病性肾病(DN)的尿蛋白排泄量、血糖及血脂的变化情况,以研究黄芪治疗DN的疗效。方法选择排泄大量蛋白尿的DN患者36例,随机分为对照组和治疗组,2组均以糖尿病饮食为治疗基础,用口服降糖药或胰岛素调整控制血糖。对照组予以卡托普利125mg,每日3次;治疗组在对照组治疗方法的基础上加用黄芪注射液20ml,加入5%葡萄糖注射液250ml并加普通胰岛素4～6单位,静脉滴注,每日1次,疗程4周。治疗前后分别检测24h尿蛋白定量、血脂及空腹血糖值,实验数据进行统计学分析。结果治疗组用药后较用药前,蛋白排泄量明显减少(P<001),对照组用药后较用药前,尿蛋白排泄量亦有减少,但无统计学意义(P>005)。2组对比降尿蛋白作用有显著差异(P<001),且治疗组血糖控制的情况也较对照组为优。结论黄芪注射液对中晚期DN可有降尿蛋白作用,具有临床实用和开发价值。

Objective Through the investigation in the changes of uric protein,blood sugar and lipoid in patients with clinical diabetic nephropathy(DN)to observe the effect of astragalus injection on patients with DN.Methods 36 DN patients with serious proteinuria were selected and randomly divided into treatment group and control group.They were all treated by diabetic diet and medicines,patients in control group received captopril 12.5mg tid po;patients in trearment group were given astragalus injection 20ml contained in 5% glucose 250ml with insulin 4～6 units through vein driping qd.Course of treatment lasted four weeks.The amount of 24 hours uric protein,blood lipoid and sugar with empty stomach was tested before and after treatment.Results Compared with that of pre-treatmeat,the uric protein in treatment group decreased greatly(P<0.01).The change of uric protein in control group was little and hadn't statistic significance(P>0.05).The effect on decreasing uric protein between the two groups had dramatical difference(P<0.01)and the level of blood sugar in treatment group was better than that of control group.Conclusion Astragalus injection can decrease the amount of uric Protein in DN patients of middle and end stage.So,it has great clinical value.
