小切口心脏不停跳下房间隔缺损修补术的临床研究(附26例报告)

Repair of atrial septal defect through minimal infrasternal thoracotomy in beating heart

目的 总结经胸骨下段小切口 ,在心脏不停跳下修补房间隔缺损 (atrialseptaldefect ,ASD)的手术结果。方法 2 0 0 0年 8月至 2 0 0 3年 9月 ,为 2 6例房间隔缺损的患者经胸骨下段小切口 ,在心脏不停跳下完成了缺损修补术。平均房间隔缺损直径 18mm ,直接缝合 11例 ,补片修补 15例。结果 无死亡和任何并发症 ,平均切口长度 (7 5± 1 0 )cm。平均体外循环时间 (3 0± 7)min ,术后平均随诊时间 (1 8± 1 0 )年。所有患者心功能NYHAⅠ级。结论 胸骨下段小切口心脏不停跳下房间隔缺损修补术 ,是一种安全、有效、美观、微创的手术方法。

Objective To summarize the results of atrial septal defect (ASD) repair minimal infrasternal thoracotomy in beating heart. Methods From Aug 2000 to sep 2003, twentysix patients were accepted ASD repair with minimal infrasternal thoracotomy in beating heart. The mean diameter of ASD was 18 mm. Direct suture of ASD was performed in 11 cases and patch repair in 15 cases. Results All patients survived without complications. Mean time of CPB was about thirty minutes. Average incision was 7.5 cm. All patients have NYHA class I function during mean 18 months follow-up. Conclusion ASD repair with minimal infrasternal thoracotomy in beating heart is not only effective and safe,but also has nice appearance.
