鼻内上颌窦扩大开窗及多鼻窦联合根治术治疗慢性多发性鼻窦炎

Treatment of multi-genesis chronic sinusitis by the expansile opening window of maxillary sinus inside nasal cavity and the united ingrained operation of multi-sinus

目的 探讨慢性多发性鼻窦炎经典手术方法的改进术式。方法 采用经下鼻道上颌窦扩大开窗途径处理上颌窦病变 ,经中鼻道清理筛窦及其它鼻窦病变的方法治疗慢性多发性鼻窦炎 (伴或不伴鼻息肉 )近千例 ,随访资料完整者 196例 ,观察疗效。结果 196例中 ,通过 1次手术治愈者 182例 (92 .86 % ) ,复发 14例 (7.14 % ) ,经第 2次手术 (11例 ,5 .6 1% )或第 3次手术 (3例 ,1.5 3% )后痊愈。结论 该手术方法治愈率高、创伤小、复发率低 ,对手术器械及设备要求不高 ,尤其适合于在基层医院广泛开展。

Objective To investigate the improved methods of classical operation of multi-genesis chronic sinusitis.Methods The pathological changes of maxillary sinus were disposed through the expansile opening window at inferior meatus,the pathological changes of ethmoid sinus and another sinus were disposed through the middle meatus.This method was adoptioned on nearing thousand patients who with the multi-genesis chronic sinusitis (with or without nasal polyp) and the following up data of 196 patients were perfect.Their curative effect were observed.Results One hundred and eighty-two cases(92.86%)of the 196 patients were cured after once operation.14 cases(7.14%)had recured and 11 cases (5.61%) of them were cured after twice operation,3 cases (1.53%) were cured after thrice operation.Conclusion The healing rate of this operation is higher,its hurt is less and recurring rate is less,the appliances and fixtures of operation are in less demand,so it especially deserves to spread in grass roots hospital.
