冬春季节儿童急性上呼吸道感染病毒病原学监测及临床研究

Pathogenic and clinical evaluation on the acute upper respiratory infection among outpatient clinic children in winter and spring.

目的 了解急性上呼吸道感染 (AURI)儿童致病的病原及其临床表现。方法 2 0 0 0年 11月至 2 0 0 3年 3月分别在冬春季节对首都儿科研究所附属儿童医院门诊 1个月至 12岁 ,发病在 3d内的AURI患儿进行咽拭子标本病毒分离、特异性血清抗体及间接免疫荧光监测 ,检测流感病毒甲1、甲3 、乙型 (A1、A3 、B型 ) ,呼吸道合胞病毒 (RSV)、腺型病毒 (ADV)及副流感病毒 1,3型 (PIF1、PIF3 )等共 7种病毒 ,并记录临床症状和体征。结果 流感病毒为AURI第 1位病毒感染病原 ,其他依次为RSV、ADV及PIF1、PIF3 。A1、A3 、B型流感病毒在冬春季节可同时出现流行 ,A1、A3 型流感病毒多发病于流行早、中期 ,B型流感病毒多发病于流行中后期。 2 0 0 0年 11月至2 0 0 1年 3月曾暴发 1次B型流感病毒局部流行。RSV、ADV在冬春季节同样处于活动状态。结论 流感病毒是冬春季节儿童AURI的主要病毒感染病原 ,同时RSV及ADV亦处于活动状态。

Objective Etiology and clinical study on acute upper respiratory infection (AURI) among children.Methods Swab virus isolation and indirect immunofluorescence to identified pathogens were done among the out patient clinic children aged 0～12 years,who suffered from diseases under 3 days and clinical diagnosis were recoded,in 3 concatenation respiratory infection seasons.Results Influenza virus was the first infection pathogen of AURI in children,followed by respiratory syncytial virus (RSV),adenovirus (ADV) and PIF virus type 1～3.Influenza A 1,A 3 and B virus were simultaneously epidemic in winter and spring,A 1 and A 3 virus occurrence at the earlier period of epidemic,and influenza B virus at the later.It was epidemic outbreak in local area from Dec,2000 to Mar,2001.RSV and ADV were also common causes of child AUPI at the same season.Conclusion Influenza virus is the most common pathogen of child AUPI in winter and spring in Beijing.RSV and ADV are also susceptible infection at the same season.
