医院感染常见革兰阴性杆菌体外耐药监测与分析

摘要 目的调查分析我院医院感染革兰阴性杆菌的菌群分布和耐药现状,指导临床合理使用抗生素。方法3年中从各类标本中分离革兰阴性杆菌314株,用常规鉴定法或API系统鉴定菌种,用KB法进行细菌的耐药监测,并加以分析。结果我院314株革兰阴性杆菌中,分离率占第1位和第2位的分别为大肠埃希菌(271%)、铜绿假单胞菌(210%);亚胺培南对所有革兰阴性杆菌具有最好的抗菌活性,氨苄西林耐药率最高,为618%~883%,其余细菌对抗生素耐药率都有升高趋势;产超广谱β内酰胺酶(ESBLs)细菌分离率为255%。结论加强对细菌耐药性的监测,及时向临床反馈公布,为临床医师合理使用抗生素提供依据。 OBJECTIVE To explore genera distribution and resistance status of Gram negative bacteria in nosocomial infection. METHODS A total of 314 Gram negative strains were collected from clinical specimens during the last 3 years. The commonly used method, API system, and K B method were adopted to identify genera and study drug resistance. RESULTS Among 314 isolates, Escherichia coli was the first with 27 1%, and then came Pseudomonas aeruginosa with 21 0%. In drug sensitivity result, imipenem showed the greatest activity, while ampicillin showed the highest resistance rate of 61 8 88 3%. All other antibiotics showed increasing resistance trend. 25 5% test strains produced extending spectrum beta lactamases. CONCLUSIONS For offering reasonable evidence to antibiotics therapy, drug resistance inspection and in time result reporting should be strengthened.
