SSFSE-MRCP对肝外胆管梗阻性疾病的诊断价值

Evaluation of MRCP using single shot projection and thick slice technique in diagnosis of extrahepatic bile duct obstruction disease

目的评价单次激发厚层投射磁共振胆胰管成像(SSFSE-MRCP)对肝外胆管梗阻性疾病的诊断价值。方法共有64例肝外胆管梗阻性疾病患者接受SSFSE-MRCP检查,其中肝外胆管结石34例,恶性胆管梗阻30例,全部病例均经手术和病理证实。结果全部64例MRCP检查均一次成功,胆胰管显示满意。MRCP对肝外胆管梗阻程度的判断和定位诊断准确率为100%,结合常规MRI图像定性诊断准确率为89.1%。结论SSFSE-MRCP检查成功率高,操作简单快速,对肝外胆管梗阻的定位诊断准确,结合常规MRI扫描,对肝外胆管梗阻的定性诊断也有较高准确性。

Objective To evaluate the diagnostic value of MRCP using single shot projection and thick slice technique in diagnosis of extrahepatic biliary obstructive disease. Methods Breath-hold MRCP with heavy T2WI fast spin echo sequence was performed in 34 cases of extrahepatic cholelithes and 30 cases of malignant biliary obstruction.The results of MRCP were compared with surgical and pathological findings. Results MRCP examination succeeded in all 64 patients and the pancreaticobiliary duct was shown satisfactorily.MRCP accurately determined the severity and the level of extrahepatic bile duct obstruction at a rate of 100% and the evaluated the causes of obstruction in combination with routine MRI at a rate of 89.1%. Conclusion Having a high successful rate,simple and rapid in use SSFSE-MRCP accurately defines the level of extrahepatic bile duct obstruction. Combining with the routine MRI scanning for analysis,its specificity for the diagnosis of extrahepatic bile duct obstruction disease is high.
