中老年髋部骨折45例临床分析 被引量:1

MIDDLE-SENILE PATIENTS WITH FRACTURE OF HIP BONE: A REPORT OF 45 CASES

摘要 目的 分析评价 45例中老年髋骨折的临床诊治原则、方法及疗效。方法 应用人工关节置换治疗股骨颈骨折 34例 ,股骨粗隆间骨折 1 1例。其中 ,骨水泥双极人工股骨头置换 34例 ,加压鹅头钉 -滑动钢板骨固定 9例 ,加压空心螺丝钉内固定 2例。术中补血 9例 (其中人工置换 2例 ) ,平均输血 40 0ml。结果 对其中 36例患者进行随访 ,平均随访 8个月 ,手术期间无一例患者死亡 ,创口均一期愈合 ,无感染 ,住院 1 2~ 2 2d(平均 1 6d) ,36例患者髋关节功能满意 ,能生活自理。结论 应用人工关节置换、内固定治疗中老年髋部骨折疗效满意 ,但对其手术风险需进行预测 ,对相关内科疾病需预防治疗 ,掌握相应的麻醉及手术适应证 ,选择适当的手术方法。 Objective To evaluate therapeutic effectiveness in middle-senile patients with hip bone fracture. Methods Of 45 cases with hip bone fracture, 34 with femur neck fracture were treated by artificial joint replacement and 11 with intertrochanter femur fracture were treated by internal-fixed of the pressure hip nail-sliding steel plate in 9 cases and pressure hollow screw in 2 cases respectively. During operation, 9 cases were performed an average 400ml blood transfusion. Results 36 cases were followed-up(from April 2000 to February 2004,averaged 8 months), none died or shocked in perioperation,and all the wounds were healed in phase I without infection. The average hospitalization was 16 days(12~22days). Conclusion For middle-senile patients with hip bone fracture,to performed a substitutive operation with the artificial joint is a good choice,but the indication of the anesthesia and operation should be grasped.
