摘要
目的 检测E 钙黏附素 (E cadherin)在胃癌组织中的表达及探讨其与淋巴结微转移的关系。方法 对 3 0例胃癌共 85 0枚淋巴结采用细胞角蛋白 2 0 (Cytokerarin 2 0 ,CK 2 0 )逆转录聚合酶链反应 (RT PCR)扩增检测微转移 ,采用免疫组织化学染色检测该 3 0例胃癌组织中E cadherin的表达情况。结果 46.7% (14 / 3 0 )的胃癌组织E Cadherin表达阴性 ;E Cadherin表达阴性与Lau ren分型、淋巴结转移密切相关 (P <0 .0 5 ) ,与分化程度及淋巴管侵犯明显相关 (P <0 .0 0 1) ,但与性别、年龄、肿瘤位置、直径及浸润深度无关 (P >0 .0 5 )。 14例检测出淋巴结微转移的癌组织中 10例(71.4% )E Cadherin表达阴性 ,而 16例未检测出淋巴结微转移的癌组织中仅有 4例 (2 5 % )E Cad herin表达阴性 ,P =0 .0 2 6。结论 E cadherin表达减弱或消失参与了胃癌淋巴结微转移的发生。
Objective To study the expression of E cadherin in gastric carcinoma and its relationship with lymph node micrometastases.Methods 850 lymph nodes from 30 patients with gastric carcinoma who underwent gastrectomy with lymph adenetomy were detected for lymph node micrometastases by using the RT PCR assay in addition to HE staining.Cytokeratin 20 gene marker was used in this assay.The expression level of E cadherin in gastric carcinoma was examined by immunochemical technique.Results The expression of E cadherin in gastric carcinoma was negative in 14 (46.7%) of 30 tumors.The negative expression of E cadherin was significantly associated with the Lauren classification ( P = 0.026), lymph node metastasis ( P = 0.011), the grade of differentiation ( P = 0.004) and the lymphatic invasion ( P = 0.001), but not correlated with age,gender,location/diameter of the primary tumor and tumor invasion ( P > 0.05). The expression of E cadherin was negative in 10 (71.4%) of the 14 patients with lymph node micrometastases,and in 4 (25%) of the 16 patients without lymph node micrometastases ( P = 0.026). Conclusion The negative expression of E cadherin may contribute to lymph node micrometastases in gastric carcinoma.
