肥胖高血压病患者肿瘤坏死因子基因多态性 被引量:2

摘要 目的探讨肿瘤坏死因子-α(TNFα)基因多态性与肥胖高血压病患者体脂分布、血压、脂代谢和胰岛素敏感性的关系。方法对上海地区汉族人中218例超重或肥胖的高血压患者,195例正常体重、正常血压受试者,用PCRRFLP方法检测TNFαG308A,G238A,TNFαA252G多态的基因型,观察各基因型与肥胖高血压病患者体脂分布、血压、脂代谢和胰岛素敏感性的相关性。结果(1)肥胖高血压病组与正常对照组之间,各等位基因和基因型频率无明显差异;(2)各基因型之间,血压水平无明显差异;(3)但在肥胖高血压病组中,TNFαG308AGA+AA基因型者的腰围、空腹胰岛素水平、胰岛素敏感指数均明显高于GG型纯合子,分别为(95.23±8.41vs93.21±8.11)cm,(P<0.05);(8.74±2.31vs7.02±2.36)μIU/mL,P<0.05;(2.20±1.31vs1.68±1.28,P<0.05)。(4)不同基因型血清TNFα浓度无明显差异。结论TNFαG308A,G238A,TNFαA252G的多态性与上海地区汉族人肥胖相关的高血压可能无关,但TNFαG308A基因型可能具有与肥胖相关的胰岛素抵抗的遗传易感性。 Objective To explore the association between tumor necrosis factor-α gene polymorphism(TNF-α) and percent body fat(TBF%), BP, lipid metabolism and insulin sensitivity. Methods (1)Genotype of TNF-α was determined by PCR-RFLP in 218 hypertensive over-weighted/obesities patients and 195 normal weight, normal blood pressure subjects. (2)To observe the relationship between genotype of TNF-α and TBF%, BP, lipid metabolism and insulin sensitivity in hypertensive obesities. Results In hypertensive obesities and control groups ,there is no difference in the distribution and frequency of TNF-α genotypes and alleles. (2)Among the genotypes of TNF-α, there is no difference in BP. (3)In hypertensive obesity group, waist (95.23±8.41 vs 93.21±8.11) cm, fasting serum insulin (8.74±2.31 vs 7.02±2.36)μIU/mL,P<0.05;and insulin sensitivity (2.20±1.31 vs 1.68±1.28, P<0.05) of TNF-α G-308A GA+AA genotype were higher than those of GG genotype. Conclusion TNF-α G-308A, G-238A, TNF-α A252G gene polymorphism appear not to be associated with BP of hypertensive obesities in Shanghai Han residents. TNF-α gene G-308A mutation maybe associated with insulin resistance involved in obesities.
