脑静脉窦血栓形成的MRI诊断 被引量:17

摘要 目的:探讨MRI对脑静脉窦血栓形成的诊断价值。方法:6例脑静脉窦血栓形成患者,男4例,女2例,年龄20~54岁。全部行MR检查,其中4例行增强扫描,3例行磁共振静脉血管造影(MRV)检查。并与DSA结果进行对照。结果:5例血栓位于上矢状窦,1例位于下矢状窦和直窦。脑肿胀6例,脑梗塞2例。增强扫描4例出现脑静脉异常强化。MRI典型表现为脑静脉窦内正常流空效应消失,代之以异常等信号或高信号影。MRV表现为受累静脉窦呈不规则血流信号或血流信号完全消失。DSA示病变部位和范围MR所见基本一致。结论:MRI及MRV是诊断脑静脉窦血栓形成的首选检查方法。 Objective To investigate the diagnostic value of MRI in cerebral venous sinus thrombosis. Methods There were 6 cases of cerebral sinus thrombosis including 4 cases of male and 2 cases of female age ranged from 20 to 54 years old. MRI were performed in all 6 cases in which Gd-DTPA enhancement in 4 cases magnetic resonance venographyMRV studies in 3 cases. MRI and MRV images were analyzed and compared with DSA images. Results The thrombosis of superior sagittal sinus inferior sagittal sinus and straight sinus was seen in 5 cases and 1 case respectively. Brain swelling was seen in all 6 cases. And venous infarction was seen in 2 cases. Abnormal venous enhancement were found on contrast enhanced MRI in 4 cases. MRI showed the normal flow void of the dural sinus disappeared instead abnormal high signal intensities were found. The signs of cerebral venous sinus thrombosis in MRV include loss of vascular flow signal or a frayed appearance of the venous sinus. The locations and scope of these lesions in DSA were the same as that in MR. Conclusion MRI and MRV are the first choice for diagnosis of cerebral venous sinus thrombosis.
