摘要
枕颈部畸形导致该部解剖结构不稳,并可能引起脊髓压迫症。治疗上,发育畸形本身不可逆转,而局部不稳和脊髓压迫症是需要解决的两个基本问题。本组应用颈枕间减压和稳定联合手术技术,即枕骨大孔扩大、寰椎后弓切除减压和枕颈间桥式植骨融合术。共治疗35例,男性21例,女性14例,平均年龄36.5岁。平均随访20个月,植骨全部获得骨性愈合;神经功能恢复正常或接近正常12例,感觉和运动功能明显改善18例,功能有改善2例,无变化3例。本文还对畸形特点,手术操作等作了讨论。
Instability of occipito-cervical region and compression of the cervical spinal cordmay be caused by multiple deformities in anatomical structure.The deformities them-selves irreversible,but instability and compression of the spinal cord must be treated.A surgical decompression combined with stabilization was designed and performed.After enlargement of the froamen occipitale magnum and resection of the posterior archof the atlas,the bone grafting was bridged between the occipital and axis.Thirty-fivecases were treated in this way.There were 21 males and 14 females with an averageage of 36.5 years.The patients were followed-up with an average of 20 months.Bonyunion in all the cases were observed.The movement and sensory function were recov-ered in 12 cases,significantly improved in 18 cases and improved in 2 cases.Therewere no obvious change in 3 cases.The diagnosis and treatment of this deformity were discussed.
