肺恶性蝾螈瘤的病理诊断

A Case Report of Malignant Triton Tumor of the Lung and Its Pathological Diagnosis

目的:探讨肺恶性蝾螈瘤的临床病理学特点。方法:对１例肺恶性蝾螈瘤进行了免疫组化染色及文献复习。结果:本文报告的肺恶性蝾螈瘤发生在右肺上叶,大体见肿物几乎占据整个肺叶,１７ｃｍ×１１ｃｍ×８ｃｍ,鱼肉状。镜下肿瘤由两种成分组成:一种瘤细胞为长棱形,免疫表型为Ｓ－１００蛋白呈阳性表达。另一种瘤细胞为多形性,胞浆嗜伊红染,免疫表型为肌球蛋白,结蛋白呈阳性表达。结论:肺恶性蝾螈瘤是一种神经来源的肿瘤,发生在肺部十分少见。本病高度恶性,生存率极低。

ve:To study the clinical and pathological features of malignant triton tumor of the lung (MTTL). Methods: An immunohistochemistry stain method was used in a case of MTTL and the relevant literatures were reviewed. Results: The tumor was found in the right upper lobe, occupying almost the whole lobe, with the size of 17 cm ×11 cm ×8 cm. Microscopically, the tumor consisted of two types of cells, one was spindle-shape cells, with the positive expression of S - 100 protein in immunological phenotype, the other was pleomorphic cells with eosinophilic cytoplasm and the positive expression of myoglobulin and desmin. Conclusion: As a neurogenic tumor, malignant triton tumor is rarely found in lungs. This disease is highly malignant with a extremely low survival rate.
